Booking a phone appointment for New Certification - What is needed and what to do
Here are the steps that we need you to do before we can offer you an appointment for new certification:
1. Register yourself/Create a user account on the PA Medical Marijuana Program website (Medicalmarijuana.pa.gov —-> “Apply for ID Card” —-> “Adult Patient Registration”)
2. Provide us with basic documentation showing that you have a qualifying diagnosis. (The list of 24 qualifying conditions can be found on the PA Medical Marijuana Program website)
Regarding Documentation:
Your documentation could be anything from any health care provider that just states your name and a diagnosis. (Does not have to be recent). Examples: Print-out/screenshot from a patient portal, After-Visit Summary, imaging report, discharge summary, or a medical bill with a diagnosis code. Please email your documentation to us as an attachment (jpeg, pdf, png, etc.) at schedule@mmjadvocatedoc.com
Anxiety or PTSD exception:
-Of the 24 qualifying diagnoses, there are two diagnoses that can be confirmed over the phone - Anxiety disorder and PTSD - since they do not require a physical exam or lab work to diagnose. If you have either of those two conditions and do not have medical records available from your healthcare provider, please just let us know.
-After you’ve read the above information and have registered yourself on the PA Medical Marijuana Program website, please email us at schedule@mmjadvocatedoc.com and include your Patient ID# (assigned to you when you registered yourself), your full name and phone number, and your documentation of a qualifying condition (or let us know if your diagnosis Anxiety disorder or PTSD.)
